This information is being collected for monitoring purposes to help evaluate how inclusive our events are. Information provided in this section will be anonymises and sent to the Scottish Government for evaluation purposes. There is a “prefer not to say” option for all of the questions.

Data Protection Notice:
We will use the personal data provided by you in this form to:

• Register and contact you about the event selected; and
• Share your name and company/organisation name with (i) the relevant event lead(s) so they can manage the guest list for the event, send a post event survey and organise discussion groups; and (ii) with the Scottish Government and venue teams for the purposes of ID checks, safety and security; and
• Share the information provided in the monitoring and evaluation section of the form with the Scottish Government on an anonymised basis for monitoring purposes to help evaluate how inclusive the events are; and
• Send you a post event survey. The results from the survey will be received by the Scottish Government directly who will publish a report containing aggregated information.
